Steps to get an Itemized Statement:

 

Please contact the Billing Dept: (Extension 3) and leave the following information:

1. Patient Name and Date of Birth.

2. Dates of Service that you are requesting Itemized Statements.

3. Your mailing address and phone #.

​

Once we receive the above info, we will mail an Itemized Statement within 7 days.

​

Due to HIPPA Regulations, statements cannot be emailed or faxed (because we can't verify who is receiving your information.)
